Risk perception questionnaire

    Hi all,
     
    Can anyone point me into the right direction to where I can obtain the original version of the risk perception questionnaires by Johnson & Tversky (1983) and Slovic, Fischhoff, Lichtenstein (e.g. 1980, 1985)? Are they included in any published titles? I have searched in databases of journal articles and unpublished tests to the best I can but all my efforts ended in vain. 
     
    Any information will be most appreciated.
